Must be ordered directly from Guardian Property Management who requires $150.00 upfront fee to complete a questionnaire.

PLEASE BE SURE THE CHECK AND QUESTIONNAIRE ARE TOGETHER. THE PROJECT NAME AND LENDER’S EMAIL ADDRESS TO RETURN MUST BE FILLED IN . IF NOT, THE QUESTIONNAIRE CAN NOT BE PROCESSED AND WILL BE RETURNED
